I want to buy a theme, need customized calculator

I really like that calculator, but I need to customize it. I would like to know if I have the freedom to customize the calculator before buying it.
What I want
1) User has the ability to choose the genre (feminine/masculine)
2) Choose age (from 0 to 65)
3) the ability to add more family members and choose again genre and age per member.

On click get a quote,  I would like to provide a downloadable pdf with the information selected and the amount given.

1) I’ll need to integrate backend support
2) customize the form to add a secondary button (add family member)
3) ability to provide a pdf on screen.

Please, what would be your advice if I can use your template.

We can customize this calculator for free. You need to describe what you want and how it should work. As I see, you need gender, age and family members fields. But how will this affect the price.

What backend support do you want to integrate?
