Form in footer?

I’d like to put a short form in the footer. I added a shortcode from Contact Form 7, but it’s having severe formatting issues (see attachment). Before I start making a bunch of CSS changes, can you tell me if there’s a recommended way to add a form to the footer?

Please, check the footer form. I live in Belarus. But I don’t use Google Chat.
