Contact Form Default Code

Can someone post the original code from the contact form on Home Page 1? I lost it and I need the CSS and fields. The form name is “Contact form – Site”.

Got it! But I don’t get any information from the form. Probably because of the name or ID of the fields. This can be because I need to translate all texts.
